Everwell Community Nursing provides Registered Nurse-led support for NDIS, Support at Home Package (Brokerage), and DVA (Brokerage) clients with complex health needs. We work alongside families, support coordinators and providers to deliver proactive care in the community.

Many NDIS, Support at Home Package (Brokerage) and DVA (Brokerage) clients end up in hospital because small health issues are missed early. Support workers are often managing complex medical needs without clinical oversight. Hospital discharge plans can fail without the right community support.

We accept self-managed and plan-managed participants. Support coordinators, plan managers, providers and families are welcome to contact us to discuss participant needs.

We support NDIS participants (self-managed and plan-managed), older Australians on a Support at Home Package (Brokerage), and DVA clients through brokerage arrangements. NDIS services are usually claimed under Improved Daily Living (Capacity Building) or Core Supports; Support at Home and DVA funding is coordinated with your provider or DVA contact. We'll confirm the right line items before we start.
We cover all of Canberra and the surrounding ACT region, including north, south, inner and Woden/Tuggeranong. For participants just outside the ACT, please call us and we'll let you know if we can help.
We usually respond to referrals within one business day. A Registered Nurse will make contact to discuss needs, and in most cases we can complete an initial assessment within 3–5 business days. Urgent post-hospital discharges can often be prioritised.
Yes. We regularly provide clinical oversight, training and ongoing nursing support for participants in SIL settings, and we work closely with house managers and support workers to keep everyone on the same page.
Anyone involved in the participant's care, including participants themselves, family members, support coordinators, plan managers, other providers, or hospital discharge teams. Use the contact form or call us directly on 0450 270 547.
We'll call you within one business day to understand the participant's needs, confirm funding, and arrange an initial visit. From there, a Registered Nurse builds a tailored plan and we begin proactive support, with regular communication back to your team.
